External Catering Form

Terms and Conditions:

  • This Form must be completed a minimum of 3 weeks prior to the event.
  • The organization/department my cancel the event at any time, at its sole discretion, without recourse by the operator.
  • The Food Service Operator must comply with all directives of University personnel
  • The Food Service Operator is solely responsible for the management, preparation, and distribution of its food and beverage (including set-up, service, supervision, tear-down, clean up) as well as all financial obligations and liabilities in connection with or necessitated by this exception permission.
  • The Food Service Operator is responsible for providing any and all equipment and personnel necessary to conduct its business.
  • The Food Service Operator shall, indemnify and hold harmless the University, and it officers, agents, and employees, from and against any and all claims, damages, costs, losses, liabilities, or expenses, including reasonable attorney’s fees, arising out of or in any way connected with this exception including, without limitation, claims for any loss or damage to property, or for death or injury to any person or persons.
  • The Food Service Operator, at its sole cost and expense, must insure it activities, and provide the University with a certificate of insurance.
  • The Food Service Operator may not assign or delegate this permit to any third party without specific written permission of the SLU Dining and Conference Services Administrative Office.
  • This permit is governed by and interpreted under the laws of the State of New York.
  • Future permission to engage in similar events is contingent upon compliance with these terms and conditions.


REQUIRED ACTION STEPS

  • Once completed by the sponsoring organization, this form will be submitted electronically to SLU Dining and Conference Services Room Reservation Clerk.  You will receive a copy of the request.
  • The recognized student organization/university department must obtain the non-university licensed food service operator’s signature on the Request for Catering Exception Form and then send the completed form to SLU Dining and Conference Services Room Reservation Clerk, Lee Hall to obtain the necessary approval signatures.
  • The sponsor must also obtain proof that the operator possesses a current food license and meets University liability insurance requirements.  A copy of the food service operator’s certificate of liability insurance and current food license should be sent along with this completed form and returned to SLU Dining and Conference Services, Room Reservation Clerk, Lee Hall, to obtain the necessary approval signatures.
  • Once approved, you will receive electronic approval for your event.
