Immunization and Medical Record Request Instructions
INSTRUCTIONS FOR OBTAINING IMMUNIZATION AND MEDICAL RECORDS
The Student Health Service at St. Lawrence University keeps medical and immunization records in storage for seven years after your date of graduation.
If you are a graduate of St. Lawrence University (within the last 7 years) and are requesting a copy of your immunization record, please send an email request to:
Include the following information to complete your request:
- Full name (name used while at SLU)
- Phone number
- Date of birth
- Year of graduation
- Fax number (or mailing address) to forward your record to.
If you wish to obtain a copy of your medical records, please mail or fax a signed Medical Records Release form, or ROI, that can be completed at the office of the Health Care provider requesting your medical records from our office. ** Please allow 7-10 days for records to be located and faxed.
