Accident Reporting Process

Employees must:

Notify supervisors within 24 hours.

Notify Safety & Security if medical treatment is needed.

Complete the accident reporting form.

Accident Trends (2020–2026)

Ceyda highlighted:

  • An increase in reports post‑COVID reopening.
  • Near misses (orange category) are encouraged because they help identify hazards early:

“These are the ones actually we want to see because these are the near misses…”

  • Medical‑only cases (blue category) are typically minor (doctor visits).

Lost‑Time Injuries

  • Only two since Ceyda started.
  • One was very short (a day or hours), one longer.
  • Some incidents were due to campus conditions (e.g., loose brick, knee pain after a fall).

Top Causes of Accidents

  1. Slip, trip, and fall (especially winter ice)
  2. Strains and sprains (musculoskeletal)
  3. Cuts (mostly dining services)

Department Trends

  • Dining Services: High reporting rate (mostly near misses), burns, cuts, wet‑floor slips.
  • Grounds & Custodial: Musculoskeletal issues, slips, ergonomics.

How Data Is Used

  • Immediate investigation of incidents and near misses.
  • Coordination with Facilities to correct hazards.
  • Updating safety training.
  • Creating safety communications and posters.

Rolling out of new Posters on all the different things to be aware of to help Campus keep safe
