Making Accessible, Fillable PDF Forms

  1. Create the pdf correctly in Word.
  2. Open the pdf in Acrobat Pro.
  3. Run the Accessibility Check.
  4. In the report pane, open the Tags view and delete the tags.
  5. Open the Prepare Form tool. You will have to re-save the pdf and then Acrobat will auto-identify any form fields.
  6. Check to make sure the fields have been identified correctly. Remove erroneous fields.
  7. Make sure each of the fields has the correct Name and Tooltip by double-clicking on each field to bring up the properties box. Both the Name and Tooltip should be the same.
