Important HEOP Pre FY Summer Information
HEOP Pre-FY Summer Session Student Health Requirements
*All forms will be sent to admitted students*
All incoming HEOP Pre-First-Year (Pre-FY) students must submit six required health documents by June 20, 2025, to ensure compliance with New York State health regulations. Students will not be allowed to check into their room or attend classes until all forms are submitted.
Submission Instructions:
- All forms must be uploaded to the St. Lawrence University Student Health Portal: https://stlawu.medicatconnect.com/
- Forms must be completed in English and must be fully legible.
Required Documents:
General Health History Form
o To be completed by the student.
o Incomplete forms will not be accepted.
o If you have significant health issues, you may attach a separate page with additional details.
Comprehensive Physical Exam Form
o Must be completed and signed by a healthcare provider.
o The exam must be dated within 12 months of your arrival.
o NCAA athletes: The exam must be within 6 months of arrival.
Vaccination Record
o Must be completed, signed, and dated by a healthcare provider.
o If records are unavailable, a titer test or revaccination will be required at your own expense.
o An attached vaccination record is acceptable if all information is up to date and legible.
Meningitis Response Form
o Required only if you choose not to receive the meningitis vaccine.
o New York State law mandates this form be included in your health file if you decline vaccination.
Tuberculosis (TB) Screening Record (Recommended, not required)
o A PPD screening test is suggested if you plan to engage in volunteer/service work with state agencies or individuals with disabilities.
Important Reminder:
- Failure to submit all required FYP forms by May 30 could result in delays in fall housing and fall class enrollment.
- This is a New York State health compliance policy, not an HEOP/SLU policy.
- Failure to submit all required HEOP summer forms by June 20 means you will not be able to check into your room or attend classes until all forms have been received and all health requirements have been met, which is why the deadline two weeks before arrival.
