Chapter Roster Forms
Greek Chapter Roster Form - Chapters must submit this form once a semester in order to provide up to date chapter information to the Office of Student Activities and Leadership.
Chapter Roster Update Form - use this form if a membership change has occurred in your chapter (i.e. member has graduated, disaffiliated, left campus etc). Please fill out a form for each member whose status or position has changed in the chapter. If a significant amount of member changes have occurred, please fill out a whole new roster using the Chapter Roster Form.
New Member Form - complete this form after initiating new members into your chapter. If you have more than 20 new members please fill out multiple forms to include all the names of the new members who recently were initiated.

Chapter Advisor & Area Alumni Information Form - Chapters should submit this form once a year and then update the form at anytime the information includes changes.
