Registration: Withdrawal from Graduate Level Course

 

Form Guidelines

  1. Do NOT leave any blanks.  This includes your student ID number. If you don't know this, please contact the Education Department.
  2. Write the term, year, course department, number, section, credit hours and title of the course.  i.e."EDPS 538 B - 3".  
  3. Obtain the signatures of your instructor and program advisor.
  4. Sign and date your form.
  5. Return to the Registar's Office in Vilas 117.
