Course Evaluations From Other Institutions and Related Resources
In April of 2008, the Office of Institutional Research sent a call to the Higher Education Data Sharing Consortium (HEDS) and asks members for copies of course evaluation forms used at their institutions, including for purposes of tenure and promotion. Over 25 forms were collected, and relevant comments have been added to the links. The form St. Lawrence University currently uses is linked here.

National Course Evaluation Forms (these appear to be the two most widely used)
- IDEA (Individual Development and Educational Assessment from Kansas State University) - used by Macalester &, Rhodes, piloted by Fairfield, and used by Hampshire as a modified, homegrown version of the instrument

- for further information,link also to Macalester's course evaluations website;note that the IEDA Center forms are the standard course evaluations used by Macalester at the end of each semester, while aseparate teaching evaluation is used when a faculty member comes up for tenure or promotion. Tthis evaluation form is online and is completed by current/former students incl. alumni
- Student Instructional Report (SIRII) offered by ETS is used by Hope College

- Agnes Scott (survey has been piloted and recently approved)
- Allegheny College
- Beloit (the form was recentlyadopted with 3 semesters of piloting )
- Bennigton (only open-ended questions)
- Claremount McKenna asks students to complete 2 surveys per course, one scannable form and one form with open-ended questions
- Davidson (all open-ended)
- Furman - a general course evaluation form and a specific evaluation form for music
- Notre Dame University survey and rational
- Earlham (most "commonly used form; all open-ended)
- Hamline
- College of the Holy Cross
- Illinois Wesleyan University
- Lehigh University
- Occidental College (1 form for regular courses, one for labs)
- Oglethorpe University (has contracted now with Academic Management Systems to use their online CourseEvalSystem and likes it)
- Randolph College (2 open-ended questions)
- Trinity College - posted here is the standardand most commonly used form
- University of the South
- Villanova - all 29 questions are used for indivi"dual faculty reports, but only questions 22, 23, 26, 28 and 29 are used for promotion & tenure purposes
- Wellesley (primarily open-ended questions)
- Whitman College (survey can be administered as either paper&pencil survey or online)
Course Evaluation Forms with Menu Approach
- Bucknell University- uses 5 standard questions, 3 core questions from a recommended cluser, recommended supplements; see also Bucknell's course evaluation website
- Franklin&Marshall College - uses a standard form and each faculty member then selects 5 additional questions from a separate menu
- George Washington University - core of 14 questions, plus optional itmes; see GU's website
- Lawrence University uses an online form that pulls in additional questions, depending on the characteristic of the course (e.g. if writing-intensive, etc.)
- Roanoke - uses a main survey; gen.ed. courses have additional items, and instructors can add up to 10 items from a catalog; q. 1-9 are used in the tenure/promotion process
