DIRECT MAIL METHOD:

A copy of the claim form and documentation should be retained by the sender.
Fax Claim Form(s) and supporting documentation to Benefits Administration Dept. to 518-234-3026.
A fax coversheet indicating the date and number of pages being sent is required. Claim Form(s) must be completed (including signature & date) and faxed along with all supporting documentation.
Illegible pages will be rejected. You will be notified by mail of any claim(s) that are rejected. We recommend that only original documents rather than copies be faxed. All required information such as date of service, type of service, patient and provider name and amount of patient responsibility must be clearly visible.
Faxed claims for the current processing period must be received no later than 10:00 a.m. on Tuesday otherwise they will be processed in your Company’s next processing period.
Notification of SSA’s receipt of the fax will be the responsibility of the sender. A copy of the claim form and documentation should be retained by the sender.

If the above information is not provided please contact Benefits Administration at 800-322-3920. Online claims can be submitted via SSA’s website at www.ssamembers.com.
Once on the website select Members.
The first time you will need to register which will require your Company’s SSA Member Number and Zip Code, once registered future log in will only require the Username and Password you selected.
Once logged in select FSA Online Claims Submission Link
Online claims must include the supporting documentation in JPG,JPEG,PDF, BMP or GIF formats. For security reasons no TIF images will be accepted
BE SURE TO SUBMIT YOUR CLAIM
A copy of the claim form and documentation should be retained by the sender along with a printed copy of the successful claim submission notice provided when the claim is submitted properly.
Online claims for the current processing period must be received no later than 10:00 a.m. on Tuesday otherwise they will be processed in your Company’s next processing period.
