INSTRUCTIONS FOR OBTAINING IMMUNIZATION RECORDS

Per policy, the Student Health Service at St. Lawrence University keeps immunization records in storage for seven years after your date of graduation.

If you are a graduate of St. Lawrence University (within the last 7 years) and are requesting a copy of your immunization record, please send an email request to:

www.healthcenter@stlawu.edu.

Include the following information to complete your request:

• Full name  (name used while at SLU)
• Phone number
• Date of birth
• Year of graduation
• Fax number (or mailing address) to forward your record to.   

You will be notified by return email when your record has been faxed. 

** Please allow 7-10 days for records to be located and faxed.
