IMPORTANT INSTRUCTIONS FOR COMPLETING THIS FORM: To increase the likelihood that you will be placed in a First-Year Program college in which are are very interested, please follow these instructions.

Read carefully ALL of the First-Year Program (FYP) course descriptions, which precede this form in the New Student Guide.
Select the number that indicates your level of interest in each FYP course below; you must select a number for EVERY course.
Rate AT LEAST THREE of the FYP courses with a "1" ("Very Interested").
Return this form in the large envelope provided POSTMARKED BEFORE JUNE 1, 2009 or complete this form online (at www.stlawu.edu/fyp/forms/forms09.htm) and select the SEND FORM button below. You may also fax this form to 315-229-5709 BY JUNE 1.

If you follow all of these instructions and submit your form by June 1, we will make every effort to place you in an FYP college in which you are very interested. You can expect notification of your FYP placement and housing on or around July 15.
