Instructions: Please pay close attention to the First-Year Program (FYP) course descriptions that precede this questionnaire. Every effort will be made to place you in an FYP college in which you are very interested. Please return this form in the large envelope provided or just select the submit form button below no later than June 2, 2008.

NOTE: If you are requesting a specific roommate, you need to confer with your roommate ahead of time about which FYP colleges you prefer. To share a room, you both must be in the same First-Year Program college; therefore, you both must make the same selection on your FYP college preference form. Also note your roommate’s name below.

To increase the likelihood that you will be placed in an FYP college in which you are very interested, you must follow these instructions.
