Academic Information Release
The Family Educational Rights and Privacy Act of 1974 (FERPA) provides that a college or university cannot release a student’s grades or academic information to parent(s) or guardian(s) without the student’s written authorization.
Complete this form if you wish to authorize the University to release such information. Note that this does not authorize a parent to act on a student's behalf.
Note: Without a signed release form parent(s) of dependent students may obtain grades and academic information by providing the Registrar’s Office with a copy of their most recent income tax statement proving dependency status of the student.
Print out, fill in and mail to the address indicated on the form by January 3.
