Below are the St. Lawrence University Summer Sport School's Health Form and Medication Form.
ALL summer sport school participants are required to fill out and return the Health Form if they wish to attend. If ANY type of medication is needed, the Medication Form must also be filled out and returned. All medication will be administered by the EMT on site.
