This page contains links to forms which need to be filled out and mailed back to the athletic department prior to participation in an intercollegiate sport. If you intend to go out for a team, you must fill out the appropriate forms before you will be able to practice/participate. (See instructions below.) INSTRUCTIONS: Click on the link to the appropriate forms. A PDF file will open. Print the form, fill it out and mail it to:

All forms for athletes who intend to participate in a fall and/or non-traditional spring sport season, must be returned by August 1, 2009.

First Year Athletes must review and complete:
Sports Information Form 1st Year Athletes
Annual Health Form First Year Athletes - If you answered YES to any questions in this form, you must complete Form II.

Returning Athletes must review and complete:
