This evaluation form can be completed by any student organization. For those organizations that may be required to keep records of their events or must submit reports of their events this evaluation form is a useful resource The submitted forms are sent to the Office of Student Activities and Leadership. Copies of the submitted form for your records can be obtained by emailing Amy Saito, at asaito@stlawu.edu.

Excellent Attendance was greater than expected, program ran smoothly,
attendees enjoyed the experience.
Good Attendance was around the group’s expectation, program ran
well but there were a few things that were not well planned out,
attendees liked the experience.
OK Attendance was lower than expected, some incidents occurred,
some attendees enjoyed it, but others did not.
Poor Attendance was very low, major incidents occurred, many people
did not enjoy the event.
